LAS VEGAS, Nev. (FOX5) – The Clark County Fire Department has released its report on a house fire in the southwest valley that killed four family members, including two children.

It happened in October at a home near Windmill Lane and Jones Boulevard.

In the CCFD report, the fire captain arriving at the scene said anybody still inside the second and third floors of the home would not survive the conditions.

“You know, we all do our jobs, but we are all kind of shocked by this fire,” said a fire official on scene the day of the fire.

The fire started in the early hours of October 24, and, according to the report, it wasn’t fully extinguished until 14 hours later.

The report from Clark County fire investigators describes security camera footage from outside the home.

At 3:12 a.m., someone is seen exiting the front door to light a cigarette. Four minutes later, they come back into the camera’s view, no longer holding a cigarette, and go inside.

At 4 a.m., the camera captures a “sudden significant glow” in the front yard, before it stops working, and smoke is seen coming from the window.

Investigators could not rule out if the cigarette was properly discarded.

They believe the fire started in the main living area on the second floor of the home.

The report says after an extensive investigation, the cause of the fire was classified as undetermined.

A month after the fire, we spoke to a fire investigator about how they work to determine the cause—and when they rule it undetermined: “That’s where rebuilding a scene comes in, and you have to come up with a hypothesis. If I can’t have a definitive hypothesis that I end up with, that I can’t disprove, I have to go with undetermined.”

The report also says there’s no indication the fire was intentionally set.
